Ischialgia Classifications

Ischialgia divided into three, namely :
 
1 . Ischialgia, as a manifestation of primary neuritis ischiadicus
Ischialgia due to neuritis is the primary ischiadicus when ischiadicus exposed nerve inflammation process. The main signs and symptoms of primary ischiadicus neuritis is pain that is felt starting from the sacrum and hip joint area, precisely at the foramen infra piriformis or incisura ischiadica and spread throughout the trip and subsequent nerve ischiadicus peroneal and tibial nerve.
Tenderness was found in and along the incisura ischiadica spasium popliteal at the acute stage. Also archiles tendons and muscles peroneus longus and tibialis anterior pain in emphasis. Muscle weakness is not severe pain throughout the leg. Because of the pain in the leg flex, when straightened pain intensified. Signs of compensatory scoliosis is common in this type ischialgia.

        Diagnosis of primary neuritis ischiadicus set when tenderness in the anterior tibialis muscle and peroneus longus. And the secondary neuritis tenderness along the nerve ischiadicus, but near the trapped nerve ischiadicus alone. Acute pain arises and is not accompanied by pain in the lower back is the primary characteristic different from ischialgia neuritis caused by problems discogenic. Archiles tendon reflexes and knee tendons are usually not disturbed.

 
2 . Ischialgia as the embodiment of entrapment radiculitis or radiculopathy
       In ischialgia radiculopathy is a result of entrapment by the tumor, which break down the nucleus pulposus into the vertebral canal and osteophytes or inflammation (spondylitis rematois angkylopoetica, herpes zoster, tuberculosis ) that are supressing, ensnare and so  radiculopathy happened.

       The general pattern is ischialgia like toothache pain or severe pain that is felt starting from the lumbosacral vertebrae and spread by ischiadicus nerve and subsequent trip to the peroneal nerve or the tibial nerve. The further to the edge of the growing pains are not so great , but paresthesias or hipoastesia often perceived.

        Anamnestic data that are common include: lower back pain always preceded ischialgia, activities that lead to elevation of intra- spinal pressure such as coughing, sneezing and straining the presence provokes ischialgia, trauma factors almost always be traced, unless neoplasmik or infectious processes responsible. As for non-physical diagnostic data of a general nature are : the lumbosacral lordosis curves are flat, showing fixation of the lumbosacral spine, tenderness on one segment of the lumbosacral spine is almost always found, lasegue test is almost always positive in less than 70 degrees, and Valsalva almost tesr naffziger always positive. Anamnestic data and diagnostic -specific physical means information that directs to a particular type of pathological process or reveal the location in the lumbosacral vertebrae or the topography of the lesions that stimulate root.
 

3 . Ischialgia as the embodiment of entrapment neuritis
         Ischiadicus nerve elements presented by spiral L4, L5, S1, S2 and S3 are preparing the lumbosacral plexus in facies pelvina os sacrum. There sacroilliaca plexus crosses the joint line and slightly more distal nerve ischiadicus form, which is the largest peripheral nerves. Furthermore, on his way to the edge of the nerve ischiadicus be trapped in buildings in its path. In the lumbosacral plexus can be infiltrated by cells of ovarian carcinoma, uterine carcinoma or sarcoma retroperineal . In line sacroilliaca joints lumbosacral plexus components being formed ischiadicus nerve can be involved in the inflammatory process (sacroiliitis). In the infra piriformis nerve foramen ischiadicus bursitis can be trapped by the piriformis muscle. In the next stretch ischiadicus nerve can be involved in major bursitis around trochantor femoris. And on the same trajectory , ischiadicus nerve can be impaired by the presence of propagation or metastasis of prostate carcinoma that has been lodged in tuberus ischii.
